The Eye Care Institute partners with the Western Diabetes Institute and every patient referred to WDI for their initial cross-disciplinary evaluation undergoes a comprehensive eye health assessment to check for any complications related to their diabetes. Special photographs are taken of the back of the eye (also called the fundus or retina) to document the health of the patient’s eyes and serve as a baseline from which to monitor over time. Eye exam findings are discussed with the patient and at the cross-disciplinary team meeting where a personalized care plan is designed to prevent or reduce the progression of diabetic eye disease, or intervene with referral to a retina specialist. Eye care is integrated into each patient’s care plan and coordinated by the WDI staff.
