  • The Western Diabetes Institute (WDI) was founded in 2009 as an integrated practice unit, or IPU, organized around people living with diabetes and related conditions, complications, and circumstances — sometimes referred to as “multimorbidity”. WDI consists of a dedicated team of experts representing a spectrum of disparate disciplines all aligned to address the complex medical and social needs of each patient.

    The WDI’s “whole-person” care model begins with a half-day, multidisciplinary evaluation in our ‘one-stop shop’ that establishes a baseline comprehensive diagnosis, which is then visualized with the aid of the Diabetes Cross-Disciplinary Index© (DXDI©):

  • On the following Friday morning, the entire WDI IPU team discusses the evaluation’s findings and together formulates a precise and personalized care plan that is aligned with the patient’s own goals. The DXDI© ‘scorecard’ enhances collective understanding and engagement, and facilitates cross-team communication through a common language.

    As an academic health center, the Western Diabetes Institute trains WesternU students in complex chronic care delivery by providing ‘real-world’ opportunities for longitudinal, experiential learning. Students are paired with a patient being evaluated in the IPU’s one-stop shop and works closely with the care coordinators/promotoras de salud. This unique student-patient-coordinator pairing is designed to promote a partnership that will lead to better understanding of the challenges and complexities of living with, and caring for, diabetes and multimorbidity.
