Information about the WDI Closure


We regret to inform you that effective August 1, 2018, the Western Diabetes Institute (WDI) located at 795 E. Second Street, Suite 4, in Pomona, California has closed and our WDI health care providers will be relocating to Western University Medical Center in Suite 5.  These providers include Dr. Erene Nawar, our Endocrinologist; Melanie Barbee, our Registered Dietician; and Crystal Pham, our Clinical Pharmacist.  As of August 1, 2018, these providers will continue to be available to provide services to you in Suite 5 which is just down the hallway from the WDI.  To make an appointment with any of them, please call 909-865-2565

 If you received services from a dental hygienist at the WDI in connection with your treatment at the WDI and you currently do not have a have a dental home (i.e., regular dentist), you can be screened at no charge by the Dental Center at WesternU located on the 3rd floor in Suite 8.  The dental screening will be performed to determine if you are eligible to receive dental treatment in our Dental Center.  The Dental Center provides patient care and education in a modern and patient-centered facility.  Dental students work under the supervision of licensed dental faculty to deliver high-quality, comprehensive oral health care.  To make an appointment for a dental screening, please call 909-706-3910.

 If you wish to obtain a copy of your WDI medical records or arrange for a copy of your WDI medical records to be sent to your primary care physician, please complete and sign the health records release authorization form.  Please feel free to contact the Western University Medical Center at 909-865-2565 if you have any questions or need any assistance.
