As an essential component of the care delivery team, the clinical pharmacy unit serves as a vital resource to improve diabetes self-management education, medication adherence, and the implementation of a precise and personalized treatment plan across the full cycle of care. The clinical pharmacist proactively communicates with the patient’s neighborhood dispensing pharmacist to assist in delivering integrated and coordinated care that is of particular importance for those patients who live with multiple health conditions and complex medication regimens.
